Background

The agency traces its roots to 1971, when the North Carolina General Assembly created the Department of Human Resources under Governor Robert W. Scott to consolidate 31 separate state bodies covering public health, social services, mental health, welfare, and related functions. It expanded steadily through the 1970s, adding the Division of Vocational Rehabilitation (1973), Office of Rural Health (1974), and Division of Aging (1977). The department was later reorganized and renamed the Department of Health and Human Services. Today it employs more than 18,000 people and manages a roughly $38 billion annual budget, making it one of the largest state agencies in the Southeast. Since January 2025, it has operated under Secretary Dev Sangvai, a physician and former Duke Health executive appointed by Governor Josh Stein (D). Under the second Trump administration, NCDHHS has been publicly vocal about threats to Medicaid and SNAP funding — in 2025 a federal court order was needed to release withheld SNAP funds to roughly 1.3 million North Carolinians, and in April 2026 Governor Stein signed emergency Medicaid bridge funding after the agency warned the program would run out of money without it.
